We have recently introduced a Clinipad system for entering your personal and medical history information.
When you arrive for an appointment you will be handed a tablet device on which will be your personal information and medical history.
Using the keyboard and clicking on a series of boxes you can review, edit or add to any information we currently hold.
The medical history is presented as a series of questions and tick boxes similar to our existing paper form. You will need to fill this form in on your next visit but each subsequent visit will be pre-populated using your previous answers.
You will also need to sign the bottom of the form using your finger or stylus provided.
Our reception team will be on hand to help should you require any assistance.

Why have we made this change ?
To comply with regulations we need to show that you review, update and sign your medical history regularly. The Clinipad system eliminates all the paper which would otherwise be generated. It also ensures our information is up to-date which is important for your safe care and gives you an opportunity to see what information we hold.
We appreciate your patience and cooperation whilst we implement this new system.
No information is stored on these tablets. When forms are completed they are transferred automatically into our Practice Management Software over a secured network connection. Our Practice Management System is protected by all the usual Information Governance protocols.
